What are all the steps and procedures in doing Nikah ? Does one need to have an Imam present ? And is a wedding required ?

For the girl and the boy, what is the marriage formula for permanent marriage?

If the two parties are going to recite the formula themselves, the woman says:
زَوَّجتُكَ نَفسي عَلى الصِّداقِ المَعلوم
[Zawwajtuka nafsi `alal-sidaq il-ma`loom]
“I made myself your wife for the specified dowry”.
And the man says:
قَبِلتُ التَزويج
[Qabilt ut-tazwij]
“I accepted the marriage”.
Notice that for a marriage to be concluded it is conditional that if the girl is virgin, it should, by obligatory caution, be contracted with the permission of her father or paternal grandfather. Without the said permission, the marriage contract is – by obligatory caution – not in order.

Imam should be present, as well as witnesses. However, the wedding and honeymoon are completely optional and it’s only up to the two of the married to decide on that. I’ve seen a marriage where a couple got married straight after the Friday prayer and there was no reception (wedding) of any sort afterwards, they made it very simple. It all really depends on how much money you have and willing to spend.

Imam and witnesses aren’t obligatory.
