Salam Brother, I am not a scholor of Islam but I will try to answer your question to the best of my knowledge.

Imam Hasan's daughter (don't know the name) married Imam Ali Zainul Abedin (Son of Imam Hussain). There after the lineage of Imam Ali Zainul Abedin continued and it is till today. So we can say that all the Imam's that came after Imam Hasan and Imam Hussain are from their lineage.
